Sartanos 20mg (Telmisartan)
Telmisartan is an antihypertensive agent and an angiotensin II receptor (type AT1) antagonist. It displays high affinity for the AT1 receptor subtype, displacing angiotensin II and reducing plasma aldosterone levels. Telmisartan acts specifically on the renin-angiotensin system without affecting other receptor subtypes or ion channels. It does not inhibit ACE (kininase II) and does not exhibit bradykinin-related side effects.
The use of telmisartan is attractive to athletes because it reduces left ventricular hypertrophy (LVH) and visceral fat. LVH, a response to intense weight training, can be amplified by steroid use. Reducing visceral fat is associated with a lower cardiovascular risk. Telmisartan acts as a harm reduction strategy, reducing overall mortality and cardiovascular disease risk.
Pharmacokinetics:
When taken orally, telmisartan is rapidly absorbed from the gastrointestinal tract with a bioavailability of 50%. Steady-state plasma concentrations reach after 3 hours, regardless of food intake. Telmisartan is highly protein bound (99.5%) and is metabolized by conjugation with glucuronic acid. The metabolites are pharmacologically inactive. The half-life of telmisartan is greater than 20 hours and it is excreted unchanged through the intestine.
How to use:
For adults, the recommended daily dose is 20-40 mg once daily, with some patients responding well to 20 mg/day. The dose can be increased up to 80 mg/day if necessary. No dose adjustments are necessary for patients with impaired renal function or elderly patients. Patients with impaired liver function should take a daily dose of 40 mg.
Athletes can use daily doses of 20–40 mg to reduce the risk of atherosclerosis, cardiovascular disease, and stroke in bodybuilders who use steroids and have prehypertension. Telmisartan may also improve HDL cholesterol levels, insulin sensitivity, mitochondrial activity, endothelial function, and cognitive function.
